About the Role:
Assist the team in diffusing current crisis with client and family, ensuring the linkage to needed
community resources, providing self-help and living skills training for the client and working with the caregivers in the implementation of home-based behavioral supports. Also assist the team in providing crisis response on a 24/7/365 basis for the client and family.
Training & Education Requirements:
Position qualifications: Bachelor’s degree with a minimum of 4 years of study. Minimum 2 years of post-bachelor’s degree experience in mental health, developmental disability, substance abuse child and adolescent population. If the Bachelor’s degree is in a non-human service field, must have a minimum of 4 years post-bachelor’s degree experience with the MH/DD/SA child and adolescent population.
Training requirements: Agency Orientation, Client Rights, HIPAA Compliance, Blood borne Pathogens, Medicaid Fraud and Abuse, Technology and Equipment, Medication Administration, North Carolina Interventions (Part A), First Aid/CPR, Documentation and IRIS system, Head Heart and Hands model, Cognitive Behavioral Therapy, Person Centered Thinking, Introduction to System of Care, Service Definition Elements, Crisis Response, Person Centered Planning Instructional Elements, Motivational Interviewing.
Job Duty Description:
Participate in the Person Centered Planning process with the client and families served.
Develop and maintain the client’s person centered plan in accordance with state guidelines. PCPs must be completed to ensure timely filing and approval of service authorizations.
Complete the authorization process for services as requested and needed. Authorizations must be submitted to the authorizing agency 15 days prior to the lapse date of the current authorization, or 15 days prior to the estimated service start date.
Provide direct home-based behavioral health services and supports, including therapy for the family. Minimum of 8 visits per week.
Provide indirect services for the family as indicated in the PCP.
Assist the team leader with consulting identified non-medical providers, engaging community and natural supports including their input in the person centered planning process.
Case management and linkage of clients to additional supports and/or services as needed.
Participate in staff meetings as scheduled.
Participate in Child and Family Team meetings as scheduled. Coordinate and facilitate CFT meetings every 30 days per agency guidelines.
Participate in the after-hours emergency response rotation.
Develop weekly schedules for visitation appointments on behalf of all team members and follow-up to ensure visitations were completed.
Complete appropriate and timely documentation of all services provided. Service notes are to be completed within 24 hours of the event.
Participate in the Quality Improvement program including health and safety.
Display good communication and feedback within a team dynamic. Professionalism in all areas of work, including client and co-worker relationships.
